- The plan was not on site, the crew improvised for field conditions, or an outdated plan revision was used.

- The approved plan is the compliance standard; a setup that departs from it is unverified and may be unsafe.
How It Is Identified
- An inspector compares the installed setup against the current approved plan and finds discrepancies.
How It Is Corrected
- Bring the setup into conformance with the approved plan, or, for a genuine changed condition, revise and re-approve the plan first.
How It Can Be Prevented Upstream
- Keep the current approved plan on site, build to it, and route changes through formal revision.
